Case Review
$1,000
For patients and families with a stack of records and no clear picture, or who want an expert read before deciding how deep to go. The full $1,000 is credited toward a Clarity Report or Molecular Strategy purchased within 30 days.
What you receive
- Independent review of pathology, imaging, labs, treatment history, and any genomic or ctDNA reports
- Written case summary (2 to 3 pages) in plain language
- Testing gap check (biomarker panels, germline testing, MSI/MMR, HER2, and others)
- Prioritized list of questions for your oncologist
- 45-minute video consultation
- Recommendation on whether a Clarity Report or Molecular Strategy would add value

Clarity Report
$2,000
For patients with NGS, ctDNA, or complex pathology results who need them interpreted, not just summarized. Everything in Case Review, plus:

- Variant-by-variant interpretation of your NGS or ctDNA report, with germline and clonal hematopoiesis flags
- Interpretation of TMB, MSI, HRD, fusions, and co-mutations
- Mapping to standard-of-care and guideline-supported options
- Additional testing recommendations with rationale
- Written Clarity Report (typically 8 to 12 pages) with a one-page executive summary
- Physician discussion guide
- 60-minute review call
- One round of written follow-up questions within 14 days

Molecular Strategy
$3,000
For patients facing a treatment decision, progression, or limited standard options, where clinical trials and treatment sequencing matter. Everything in the Clarity Report, plus:

- Clinical trial shortlist (typically 5 to 10 trials) screened against your biomarkers, prior treatments, and eligibility criteria
- Treatment sequencing map
- Monitoring strategy for ctDNA/MRD, imaging, and re-biopsy triggers
- Physician-facing summary letter
- 90-minute strategy session
- 30 days of email follow-up

Who owns my case between appointments?
Active Case Management
Starting at $7,500 per month
Ongoing ownership of your case between appointments. Flat, all-inclusive monthly fee, month to month, no minimum commitment.

- Continuous case ownership
- Every new scan, lab, and molecular result analyzed as it comes in
- Full clinical trial search and placement included
- Direct correspondence with your oncology team
- Scheduled calls and direct phone and email access

Surveillance
You are finishing Active Case Management, or you are in remission or on watch-and-wait and want the case watched closely.
Surveillance Program
Surveillance membership, from $1,000 per month, paid annually ($12,000), with 6-month, quarterly, and monthly terms
Ongoing interpretation of serial ctDNA/MRD results (Signatera, Guardant Reveal), scan and lab review, a standing check-in, and priority access if something changes. Built as the step-down from Active Case Management, and equally suited to patients in remission or on watch-and-wait who want a maintained baseline watched over time. This tier pairs with the Survivorship and Surveillance approach on /survivorship.
What you receive
- Ongoing interpretation of serial ctDNA/MRD results, including Signatera and Guardant Reveal
- Scan and lab review as results arrive, read against your own baseline rather than one report at a time
- A standing check-in each quarter, with threshold and trend flagging between
- A written quarterly surveillance summary with questions for your next appointment
- Brief questions answered between quarters by text or email
- Priority access if something changes, with immediate step-up into Active Case Management and the remainder of the quarter credited

Built as the step-down from Active Case Management, and equally a fit for patients in remission or on watch-and-wait, such as SMZL and follicular lymphoma, where the point is catching a change against your own baseline early. A Clarity Report or Molecular Strategy alone does not establish that baseline; surveillance enrollment follows the free consultation. Memberships renew on the same term; cancel with notice before your renewal date.
Concierge
You want one accountable scientist across the entire case, at the speed the disease moves, with the logistics handled alongside the science.
Private Client Advisory
Starting at $10,000 per month
The concierge tier. Everything in Active Case Management, plus priority response, family briefings, second-opinion and records coordination, and full clinical trial search and placement included. - Second-opinion coordination: identifying the right outside institution, approaching it, and preparing the records package
- Records gathering and organization across institutions, handled for you
- Flat monthly fee, no hourly billing, month to month with no contract term
